STEPS:

  1. Injured Worker notifies Supervisor.
  2. Supervisor or Injured Worker immediately calls injury reporting hotline.
  3. Nurse gathers information over the phone and helps Injured Worker access appropriate medical treatment. 
  4. Supervisor provides claim form to Injured Worker
  5. Supervisor completes Supervisor Incident Investigation Report and submits to Risk.
